What should I bring to my first appointment?
Please bring your up-to-date insurance cards, photo identification, and medication list. Under the “Patient” tab, you will find paperwork under “New Patient” that we encourage you to fill out prior to your visit. Depending on when your appointment is scheduled, paperwork may be sent directly to your home. If you do not receive your paperwork prior to your office visit or if you do not fill it out before, please arrive 15 minutes prior to your appointment.
Does OOA participate with my insurance plan?
We have live operators that can assist you with understanding your benefits Monday-Friday. Please call (732) 349-8454.
My appointment was made with a PA. What is a PA?
A Physician Assistant, also known as a PA, is a healthcare professional that is nationally certified and licensed to practice medicine. They are crucial members of the healthcare team that work closely with physicians to care for patients in every field of medicine. PAs complete a rigorous 27-month program that is made up of a didactic year, followed by 15 months of clinical rotations in all different specialties. PAs can obtain medical histories, conduct physical exams, as well as diagnose and treat patients. They can also order and interpret diagnostic tests, perform procedures such as intra-articular injections, counsel patients on preventive healthcare, assist in surgery, and write prescriptions. They are vital to our practice in helping us deliver the highest quality of care to our patients.
What should I bring to my MVA or Workers Comp appointment?
Please bring the name of the insurance, date of the accident, claim number associated with the case, assigned adjustor’s name and contact information.
What is OOA’s Medication Refill policy?
If you require a prescription refill, we ask that you please give us at least 72 hours notice. If you have not been seen by your doctor in the last 3 months, an in-person appointment will be required in order for the physician to refill your medication.
How do I obtain my medical records?
You may request your medical records by filling out our “Patient Request Form,” which can be found under the “Patient Info” tab. You could also stop by our office, or give us a call at 732.349.8454. If you are requesting records be sent to another physician’s office, there is no charge to fax these records. If you would like a copy of your medical records for your files, there is a fee of $1.00 per page and $10.00 for an X-Ray/MRI disc. Unfortunately, this service is not covered by insurance and payment is due before copies are made. Please allow up to 30 days for your records to be prepared. Every attempt will be made by our office to expedite this process. You will receive a call when your records are ready to be picked up. Thank you in advance for your patience!
