Please bring your up-to-date insurance cards, photo id and medication list. Also under the "patient" tab you will find the paperwork for "new patient" that could be filled out prior to coming in. Sometimes depending on when your appointment is scheduled, paperwork maybe sent directly to your home. If not please arrive 15 minutes prior to your appointment.

MY APPOINTMENT WAS MADE WITH A P.A., WHAT IS A P.A.?
A Physician Assist (P.A.) is nationally certified and licensed in which they completed a program for 26 months with a minimum of 2,000 hours of clinical hours. PAs can obtain medical histories, conduct physical exams, diagnose and treat, order and interpret tests, perform medical procedure like joint injections, counsel patients on preventive healthcare, assist in surgery, write prescription and make rounds in nursing homes and hospitals, among many other medical services.
WHAT SHOULD I BRING TO MY MVA OR WORKERS COMP APPOINTMENT?
Please bring the name of the insurance, date of the accident, claim number associated with the case, assigned adjustor's name and contact information.
WHAT IS OOA'S MEDICATION REFILL POLICY?
Upon requiring a refill, we ask for 72 hours notice. If you have not been seen by your doctor in the last 3 months, an appointment will need to be scheduled in order for the refill to be prescribed.
HOW DO I OBTAIN MY MEDICAL RECORDS?
You may obtain your medical records by filling out our "patient request form" which can be found under the "patient" tab, by stopping by our office or by calling 732.349.8454. While requesting records for another physician there is no charge to fax. If the request is for personal records there is a fee of $1.00 per page and $10.00 for an x-ray/MRI disc. This is not covered by insurance and payment is due before copies are made. By Law, please allow up to 30 days to be prepared. Every attempt will be made to expedite this process and you will receive a call when your records are ready to be picked up.
